Stem to Scrubs: Youth Medical Simulation Camp

🔒Consent Policy

Participation in the STEM to Scrubs: Youth Medical Simulation Camp requires informed consent from a parent or legal guardian. The following conditions apply:

• Parents/guardians must fully complete the registration and waiver form.

• Participation is contingent on the student being medically able to engage in physical and educational activities.

• Program materials and interactions are designed for educational purposes only. No diagnostic, therapeutic, or clinical services are provided.

• All instructional content will be presented in a developmentally appropriate, respectful, and inclusive manner.

• Staff members are trained in youth engagement, safety protocols, and emergency response procedures.

• Any medical concerns or conditions must be disclosed on the registration form. This includes allergies, physical limitations, and relevant behavioral conditions.

• Participants are expected to follow all safety instructions and behavioral guidelines. Disruptive or unsafe behavior may result in removal from the program at the discretion of staff.

• Photos and video may be taken for promotional use unless the parent/guardian opts out in writing.

• Addison Institute reserves the right to dismiss any student from the program if they are disruptive, aggressive, or jeopardize the safety or learning experience of others. No refund will be issued in such cases.

• In the event of an emergency, Addison Institute staff will take reasonable steps to provide appropriate care and contact the parent/guardian or emergency contact listed.

• Consent may be withdrawn at any time in writing; however, no refunds will be given after the first day of the camp.

By submitting the registration form and participating in the program, you acknowledge that you have read, understood, and agree to the above policies and terms.
