Tuition and Financial Responsibility

I understand the current in-person CPT tuition is $2,275 unless a written scholarship, sponsorship, or approved payment arrangement applies. I am responsible for charges shown before payment is submitted.

Cancellation and Refund Policy

I understand the three-business-day cancellation right stated in the enrollment agreement and that, after attendance begins, refund calculations for a program of 300 hours or less may be based on a weekly pro-rata tuition calculation under applicable New Jersey private-career-school rules.

Supervised Venipuncture / Capillary Collection Informed Consent

I understand hands-on training may include supervised venipuncture and capillary collection. Possible risks include pain, bruising, bleeding, hematoma, fainting, infection, nerve irritation/injury, allergic reaction, and accidental blood or sharps exposure. I may stop an individual procedure and immediately notify the instructor of a safety concern.
